Additional Charges May apply for CACI certification and Special Issuance

In most cases the charge will be $30 and $75 respectively.  The fee is based on the time involved in reviewing correspondence, documentation, and communication with other treating physicians and/or the FAA.  If you are on a Special Issuance or if you have a recent correspondence with the FAA, please bring all FAA paperwork regarding your situation with you to the appointment.

DO NOT FLY:  Review FAA’s list of prohibited medications.  Do a Google search for “FAA Medical and (name of medication)” regarding any medication which your doctor may consider prescribing.  The search will indicate if your medication is considered “not acceptable”.  If you have questions, you can discuss them at the time of your visit with Dr. Cromer.

If you are applying for a Basic Med

Go to FAA.gov and carefully follow the checklist for Basic Med.  This will result in you printing and bringing the medical application with you to your appointment.

The Basic Med certificate will be based on criteria very similar to that of the FAA in determining whether or not you qualify to hold a normal or special issuance 3rd class medical certificate.

Because Dr. Cromer is solely responsible for the medical decision, based upon FAA criteria, he will require thorough documentation re:  any health problems you have had as well as a history of medication use.  If you have a significant medical history, for example, heart, neurological or psychological issue, please bring status information from a treating doctor.
